Step-by-step instructions to your expedited
Third Party Pay medical diagnostics.

If you need an MRI, CT or CTA scan as soon as possible, at a convenient time and location, follow these simple steps: 

  1. Phone Laura at Axxess Imaging at 416-886-0304 OR download and complete the hospital Referral Package below OR fill out the Diagnostic Services Form at the bottom of this page.

  2. For the quickest MRI & CT Scans download MRI Referral Package or download CT Referral Package.

  3. For CTA Scans, please contact laura.miller@axxessimaging.com directly for a complete package to be emailed to you.

  4. Have an Ontario Medical Doctor complete "Doctor's Referral Form" (page 1 of Referral Package).

  5. Complete the "Third Party Payor Information Form" (page 2 of Referral Package). The patient can NOT be the corporate contact person.

  6. Complete the "Declaration of Non-OHIP/Third Party Eligibility & Waiver Form” (page 3 of Referral Package).

  7. Scan and email all pages to laura.miller@axxessimaging.com OR FAX all pages to 1-888-391-3608.

  8. Axxess Imaging will begin processing immediately .

  9. Axxess Imaging’s Intake Coordinator will contact the patient directly to book either your MRI within 2 to 3 days, your CT within 5 days or your CTA within 5 days.

  10. Axxess Imaging’s Intake Coordinator will make payment arrangements with the third party payor.

  11. After the MRI/CT scan the patient will have access to view and share their MRI/CT images on PocketHealth.

  12. A radiologist diagnostic report will be faxed to the referring doctor and third-party private payor (if requested) within 24 to 48 hours from the time of your scan.
